星空影视Staff Library Account Request Form
This form is for 星空影视staff members. The purpose of this form is to send a request to 星空影视Library to have a library account set up for you.
Once it has been set up, you will be able to access 星空影视Library services and our licensed online databases using your My星空影视login. If you don’t have a My星空影视login, you will need to contact the IT Help Desk.
Once you have a MyAu login and you submit the form, a library staff member will contact you.
Note: The personal information collected on this form will be used for the purpose of processing your request for the assistance from 星空影视Library. This personal information is collected under the authority of the Post Secondary Learning Act that mandates the programs and services offered by Athabasca University, and will be protected under the privacy provisions of the Alberta Freedom of Information and Protection of Privacy Act.
